    From Dr Sandra Fryhofer of Medicine Matters. Cervical artery dissection is serious. It's most prevalent in the upper cervical spine and can involve the internal carotid or vertebral arteries. It's an important cause of stroke in both young and middle-aged patients. Dissections can be spontaneous or traumatic from causes as varied as a high-speed car crash to a sporting activity such as heavy weightlifting, even golf, tennis, or yoga, to ordinary activities such as sneezing and coughing.     Okay. Real story. Real patient. My patient. A new patient, a middle-aged executive, comes in with severe neck pain and headache worse than any headache he's ever had before. Earlier that day he'd seen a chiropractor. The diagnosis was left vertebral artery dissection, revealed by CT angiograms. My thought at the time was concern that manipulation by the chiropractor probably had a role. That was 10 years ago.

    Neck pain is common, especially because we spend so much time typing on computers. Neck pain has many going to massages, maybe seeing chiropractors. But the lesson I learned from my patient is reinforced by this study. Be gentle with your neck.   • Study: Mediterranean Diet May Help Slow Aging Process

    Could the foods we eat could slow down or speed up the aging process? And if so, what foods potentially offer a fountain of youth and why? According to a new study published in The British Medical Journal, eating a Mediterranean diet (see sidebar) might help extend your lifespan because it appears to be associated with longer telomere length, which is an established marker of slower aging.

    Telomeres sit on the end of chromosomes (like the plastic tips on the end of shoelaces), stopping them from fraying and scrambling the genetic codes they contain. In healthy people, telomeres shorten progressively throughout life, more than halving in length from infancy to adulthood, and halving again in the very elderly.

    A Life-extending Diet

    A Mediterranean diet typically includes:

    Large amounts of: vegetables, fruits, nuts, legumes (such as peas, beans and lentils), unrefined grains and olive oil
    Moderately high amounts of: fish
    Low intake of: dairy products, meat and poultry; saturated fats
    Regular, but moderate intake of alcohol (specifically wine with meals)
    Shorter telomeres are thus associated with lower life expectancy and greater risk of age-related diseases. Lifestyle factors, such as obesity, cigarette smoking and consumption of sugar-sweetened drinks, have all been linked to people having shorter telomeres than typically occur in people of a similar age. Oxidative stress and inflammation have also been shown to speed up telomere shortening.

    By contrast, fruits, vegetables and nuts—key components of the Mediterranean diet—have well-known antioxidant and anti-inflammatory effects. With that in mind, a team of U.S. researchers, led by Immaculata De Vivo, Associate Professor at Brigham and Women's Hospital and Harvard Medical School, set out to examine whether adherence to the Mediterranean diet was associated with longer telomere length.

    The Study

    Researchers analyzed data on 4,676 healthy middle-aged women from the Nurses’ Health Study—an ongoing study tracking the health of more than 120,000 nurses since 1976. Participants completed detailed food questionnaires and had a blood test to measure telomere length.

    A diet score ranging from 0 to 9 points was calculated for each participant, with a higher score representing a closer resemblance to the Mediterranean diet.

    After adjusting for other potentially influential factors, researchers found that greater adherence to the Mediterranean diet was significantly associated with longer telomeres. Each one-point change in diet score corresponded on average to 1.5 years of telomere aging.

    Importantly, none of the individual dietary components was associated with telomere length. In other words, no single food was responsible for the longer telomeres. Instead, researchers emphasize the importance of looking at the overall quality of a person’s diet rather than separate dietary factors such as whole-grain intake, for example.

    “To our knowledge, this is the largest population-based study specifically addressing the association between Mediterranean diet adherence and telomere length in healthy, middle-aged women,” the researchers noted in their published report. “Our results further support the benefits of adherence to the Mediterranean diet for promoting health and longevity.”

    A Mediterranean diet is the cornerstone of dietary advice in cardiovascular disease prevention, and the fact that it also links with a biomarker of slower aging is reassuring, says Professor Peter Nilsson from Lund University, Sweden in an accompanying editorial.

    He suggests that genetic background factors, reflecting ancestry, could probably explain some of the variation in the association between dietary patterns and telomere length, and that future studies on this question “should take into account the possibility of interactions between genes, diet, and sex.”

Gretchen Rubin is the author of The Happiness Project, (4) a book and a blog about her adventures learning to be happier. In a Psychology Today article she says:

    Although it’s helpful to focus on the positive, to count your blessings, and to remind yourself of what makes you happy, it’s also very important to pay attention to what’s undermining your happiness. (5)

    Rubin strongly suggests that you ask yourself what the obstacles are to your happiness. When you identify an obstacle be specific. What exactly is bothering you? Why is this having such an impact? And finally, what do your need to do to change your behavior?

    Obstacles could include:

    Equating success with money and status
    Blaming others for your unhappiness, it’s always someone else’s fault
    Being afraid
    Staying in toxic relationships
    Not being honest with yourself and others
    Focusing your life obsessively on making good impressions
    Neglecting introspection and awareness of yourself, of your values and beliefs
    Needing to make everyone else happy
    Thinking life should always be wonderful.

    When we are generous in welcoming people and sharing something with them—some food, a place in our homes, our time—not only do we no longer remain poor: we are enriched. (7)

    The modern ideals of total autonomy and radical self-reliance are profoundly misplaced. It is incumbent on us to either change or move away from toxic personal and social relationships. Our happiness and health depend on it.

    Remember, happiness comes from the inside and manifests itself in loving personal relationships and our connection with the world. It is of the utmost importance that we seek out people and communities that offer us love and understanding and to which we can offer the same. It is important that we work to see that all people regardless of ethnicity, race or sexual orientation have the opportunity to live without fear and enjoy the fruits of loving relationships.
